Medication FAQ for Parents/Guardians

What is needed if my student requires medication at school?

For any medication that cannot be administered outside of school hours (including both prescription and over-the-counter medications), the following are required:

  • Provider Order: A signed order from a licensed healthcare provider (MD, NP, PA).
  • Parent Permission: A signed consent/permission form from the parent or guardian.
  • Annual Renewal: New medication orders and permission forms must be submitted at the start of each school year.

Where can I find the necessary forms?

Forms are available via the following methods:

  • District Health Services Website: Download the "Parent & Provider Permission to Administer Medications" form.
  • School Health Office: Hard copies can be obtained directly from the school health office.
  • Provider Forms: Your provider may use their own office form if preferred, provided it contains all necessary information.

 

How do I get medication to the school?

To maintain safety protocols, please follow these delivery guidelines:

Over-the-counter medications must be unopened.

When can I drop off medication?

Please contact the school nurse directly to confirm building specific drop-off times. If you need to make special arrangements outside of standard hours, please call the School Health Office to coordinate.

Restrictions and Special Conditions

Non-FDA Sanctioned Medications

School nurses are not permitted to administer non-FDA sanctioned medications or supplements. This includes, but is not limited to:

  • Herbal remedies and essential oils.
  • Dietary supplements and natural products.
  • Naturopathic or holistic medicines.
  • Nicotine replacement therapy.

 

Self-Carry and Self-Administration

In accordance with Education Law §§ 916, 916-a, and 916-b, students may be permitted to self-carry and self-administer medication under specific circumstances:

  1. Eligible Conditions: Respiratory conditions, allergies, or diabetes.
  2. Required Documentation:
  • A provider order for the medication that also attests that the student is competent to self-administer effectively.
  • Parent/guardian permission form.

 

Important Note: Even with provider orders, medications such as general pain relievers or migraine medications are not allowed to be self-carried or self-administered.
