Nurses Notes for Students New To the District
Education Law and Regulations of the Commissioner of Education require a physical examination of all children when they enter the school district for the first time. The exam “shall not be performed more than 12 months prior to the start of the school year in which the exam is required."
Proof of immunizations is mandatory. The Baldwinsville registrar will forward immunization records to the appropriate school nurse for review. The school nurse will contact the family with any concerns in order to avoid exclusion from school based on NYS immunization requirements.
For those students entering from another state or country, please take note of the New York State immunization requirements. Details can be found on the immunization link. Families who present with foreign immunization documents must provide an appropriately transcribed record in English. Assistance with this can be obtained from the Onondaga County Health Department. Registration paperwork will require you to complete a health history and family history questionnaire.
